Primary Care Nurse

We are seeking a skilled Primary Care Nurse to join our team at the BC Centre for Excellence in HIV/AIDS' Research & Innovation Centre. As a member of our interprofessional healthcare team, you will play a vital role in providing comprehensive care to clients/patients in the Downtown Eastside.
Key Responsibilities- Conduct assessments and develop care plans in collaboration with the healthcare team.
- Triage for physical health and psychosocial needs.
- Perform primary care nursing duties, including wound care, addiction assessments, harm reduction, HIV and HCV testing, phlebotomy, diabetes care, pain management, and pregnancy and urine drug tests.
- Document information, including assessments, plan of care, patient/client status, and progress.
- Provide continuity of care by liaising with the interdisciplinary team and implementing clinical interventions.
- Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing RN registrant or RPN registrant.
- Two years' recent related experience working with diverse marginalized populations in healthcare settings or community in a primary care, HIV, addictions, and/or mental health setting.
- Completion of the Point of Care HIV Testing course and successful completion of the BCCDC Immunization certificate and Narcan Certification.
- Demonstrated experience and/or successful completion of recent education/training in performing venipuncture.
- Knowledge of and exposure to trauma-informed care, violence prevention, transgender care, harm reduction, recovery-oriented practice, and indigenous cultural competency training.
- Comprehensive triaging skills for physical health and psychosocial needs.
- Demonstrated skill in physical assessments and chronic disease management.
