We are seeking a skilled Nurse Care Specialist to join our team at RhynoCare, a leading provider of high-quality healthcare services. As a key member of our nursing team, you will be responsible for delivering exceptional patient care and support in a fast-paced environment.
Key Responsibilities:
- Provide comprehensive nursing care to patients in accordance with established policies, physician's orders, and the patient's plan of care.
- Utilize sound nursing practice principles to ensure the delivery of excellent nursing and personal care in the assigned home area(s).
- Ensure timely and accessible admissions for patients, including a comprehensive orientation to our services and facilities.
- Coordinate resident transfers and discharges in a safe and efficient manner.
- Manage and maintain accurate records of patient care, including medication administration and treatment plans.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the physical, psychological, emotional, spiritual, cultural, and social needs of patients.
Requirements:
- Bachelor of Science in Nursing (BScN) or equivalent degree.
- Current registration as a Registered Nurse with the College of Nurses.
- Basic Cardiac Life Support (BCLS) certification.
- Basic computer proficiency and ability to navigate electronic medical records.
- Valid First Aid and CPR Certificate.
- Strong written and verbal communication skills in English.
- Ability to work effectively in a team environment and provide compassionate care to patients and their families.
- Knowledge of relevant healthcare regulations and standards, including the Long Term Care Act and the Health Disciplines Act.
- Medical Clearance and a clear Vulnerable Sector Police Check.
