Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our Community Wellness team at Mount Carmel Clinic. As a key member of our interdisciplinary team, you will provide a range of client care services to ensure client engagement and reduce barriers to care.
Key Responsibilities- Provide extended community outreach and health education to clients with acute and complex care needs
- Develop and implement case management plans to promote wellness and disease prevention
- Facilitate group sessions and community programming to support client engagement and empowerment
- Advocate for clients with Indigenous communities and organizations to promote cultural safety and understanding
- Collaborate with interdisciplinary team members to provide comprehensive care and support
- Master's degree in Social Work or related field
- Minimum 5 years of experience working as a Social Worker in addictions, mental health, and crisis intervention
- Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
- Commitment to the principles of Mount Carmel Clinic, including diversity, equity, cultural safety, and harm reduction
- Experience working with Indigenous communities and organizations
- Knowledge of Indigenous culture and traditions, and the impact of colonization on Indigenous health and well-being
- Fluency in additional languages
