We are seeking a skilled and compassionate Healthcare Assistant II - Home Health to join our team at Fraser Health. As a key member of our home health team, you will provide high-quality care and support to clients in their own homes, promoting independence and dignity.
Key Responsibilities- Assist clients with daily living activities, such as bathing, dressing, and grooming, to promote maximum independence.
- Provide personal care, housekeeping, and home management services, as outlined in the client's care plan.
- Perform delegated tasks, such as catheter care and exercise routines, as trained and authorized.
- Work collaboratively with professional staff to ensure seamless care and support.
- Report any changes in client condition, behavior, or living arrangements to the appropriate staff.
- Participate in quality improvement activities and maintain current knowledge of Home Health and Home Support guidelines, policies, and procedures.
- Grade 12 diploma, plus a Home Support/Resident Care Aide certificate, or equivalent combination of education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
