Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team in Correctional Health Services. As a key member of our interprofessional team, you will provide clinical pharmacy services to clients in a dynamic and challenging environment.
Key Responsibilities- Identify and resolve actual and potential drug therapy problems in collaboration with the client and other members of the healthcare team.
- Provide comprehensive medication management and pharmaceutical care by monitoring the medical condition(s) and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions, and therapeutic, chemical, and laboratory incompatibilities.
- Communicate pertinent findings, recommendations, and plans to the client and appropriate healthcare practitioners.
- Consult with clients and members of the healthcare team to develop client-specific therapeutic plans based on chart review, client information, and assessment of clinical responses.
- Maintain appropriate documentation in the health record in accordance with organizational standards and practices as required.
- Use appropriate resources to support professional practice and client care, including those available through provincial programs.
- Establish linkages with health authority, community pharmacy services, and other relevant community services to support continuity of care for clients.
- Provide drug information, consultation, and other support services to medical, nursing, pharmacy, and other healthcare team members.
- Conduct client counselling and education regarding drug therapy.
- Provide in-service education to the healthcare team where appropriate.
- Provide instruction and supervision to students.
- Participate in various committees to provide guidance and advice on drug therapy.
- Participate in quality improvement activities through reviewing current practice, identifying actual or potential problems, and recommending and evaluating modifications.
- Maintain workload records and metrics as required.
- Participate in programs such as Patient Safety and Drug Reaction Reporting by collecting and compiling relevant data.
- Share the responsibility for orientation of new personnel and act as a role model.
- Maintain a working knowledge of PharmaCare policies and procedures.
- Current registration as a pharmacist with the College of Pharmacists of British Columbia.
- Two years' recent, relevant patient care experience or equivalent combination of education, training, and experience.
- Valid BC Driver's Licence and access to a personal vehicle for business-related purposes.
- Broad knowledge of organization policies, procedures, and standards of care.
- Knowledge of evidence-based clinical pharmacy practice.
- Knowledge of medication use evaluation.
- Knowledge of research processes and methodology.
- Knowledge of drug distribution processes in a health system setting.
- Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision.
- Ability to communicate effectively both verbally and in writing.
- Ability to establish and maintain effective working relationships with other healthcare practitioners, as well as clients, family members, and the public.
- Ability to organize and prioritize work.
- Ability to operate related equipment.
- Physical ability to carry out the duties of the position.
