Job Summary:

The Parent Partner will work closely with the Community Transition Team to understand individual family dynamics, strengths, and needed supports to best assist the family in the youth's transition home.

Key Responsibilities:

  • Identify and facilitate connections to needed services for parents and assist families in the identification of and connection to community-based resources as an alternative or as a supplement to CTT services.
  • Assist families in navigating the transition process and connecting them with relevant resources and support systems.
  • Collaborate with the Community Transition Team to develop and implement individualized transition plans for families.
  • Provide support and guidance to families as they navigate the transition process.

Preferred Qualifications:

  • It is preferred that the Parent Partner(s) have a child or relative who has aged-out of DYS.
  • Prior court-related involvement with the MA Department of Children and Families or the juvenile justice system may also be considered.

Education:

High School Diploma or equivalency
