Develops programs based on the identification of community needs and individual client interests Ensures program development is rooted in best practices and successful outcomes from similar initiatives Guarantees programs are adaptable, diverse, and can evolve to meet changing community needs Provides mentorship to personal support staff and assists with personal care for clients as necessary Identifies potential residents who may benefit from the program to the Care Coordinator Supports staff and clients regarding the safe use of key lock boxes Maintains and regularly updates program profile forms in the designated resource at each local office Collects data related to program indicators for monthly submission to the Clinical Supervisor Oversees PERS unitsQualifications
Must possess a valid Personal Support Worker certificate in Ontario Knowledge and skills related to geriatric care enhanced through clinical experience as a PSW Minimum of 3 years of experience in a healthcare or community-based environment Familiarity with community resources Exceptional coordination and organizational abilities Strong facilitation and teaching skills for group sessions Excellent verbal and written communication and interpersonal skills Proven ability to mentor and collaborate effectivelyCarePartners Commitment
In addition to delivering home-based health care, CarePartners actively engages with the community through clinics, transitional care units, and provides support in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we have been facilitating staff-led medical care and clinics in areas with limited healthcare access since 2009.
