Job Title: Care Coordinator

Are you a dedicated healthcare professional seeking a challenging role that makes a meaningful difference in patients' lives? We are seeking a Care Coordinator to join our team at Home and Community Care Support Services.

Job Summary:

The Care Coordinator will play a vital role in assessing patient care needs, determining eligibility for services, and developing individual care plans. This is a 12-month temporary full-time opportunity in the St. Thomas Elgin General Hospital team.

Key Responsibilities:
  • Collaborate with patients and their families to assess care needs and develop individual care plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
  • Ensure patients' values and preferences are respected
Requirements:
  • Membership in good standing with the applicable regulatory body
  • 2+ years of recent experience in community health or a related field
  • Knowledge of the healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization, and organizational skills
  • Established ability to accurately complete required documentation, reports, and forms
  • A valid driver's license and access to a reliable vehicle
  • Proficient in a Windows environment
Preferred Qualifications:
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
