Key Responsibilities
  • Develop and maintain effective care plans in collaboration with the Ontario Health atHome team
  • Provide one-on-one nursing services to patients in their homes, including in-person and virtual care
  • Communicate and liaise with patients, family members, and the interdisciplinary care team
  • Perform other clinical duties as required

Requirements

  • Exceptional interpersonal skills
  • Valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
