Key Responsibilities- Develop and maintain effective care plans in collaboration with the Ontario Health atHome team
- Provide one-on-one nursing services to patients in their homes, including in-person and virtual care
- Communicate and liaise with patients, family members, and the interdisciplinary care team
- Perform other clinical duties as required

- Excellent English verbal and written communication skills
