Nurse Practitioner

We are seeking a skilled Registered Nurse to join our E Wing (CCC/Pall/Rehab) Department at Headwaters Health Care Centre. As a key member of our healthcare team, you will provide high-quality patient care and contribute to our mission of delivering exceptional care to our community.
Responsibilities:- Provide general and specialized nursing duties and work assignments according to the standards of Nursing Practice of the College of Nurses of Ontario and approved standards of care of the Corporation.
- Work collaboratively with our interdisciplinary team to deliver patient-centered care.
- Develop and implement individualized care plans to meet the unique needs of our patients.
- Contribute to the development and implementation of policies and procedures to ensure high-quality patient care.
- Registered Nurse in good standing with the College of Nurses of Ontario.
- BScN preferred, minimum completion of Diploma in Nursing.
- Minimum one year of recent (within 1-2 years) clinical experience.
- Current BCLS certification.
- Previous experience in palliative care an asset.
- Leadership experience an asset.
- Certificate in Gentle Persuasive Approach (GPA) preferred.
- Recent Palliative Care and Rehabilitation Experience preferred.
- Canadian Nurses Association (CNA) Gerontology certification preferred.
- LEAP training preferred.
- Acceptable performance and attendance records.
