Job Title: Primary Care Community Representative

Job Summary:

We are seeking a highly motivated and compassionate individual to join our team as a Primary Care Community Representative. In this role, you will provide support to new and young mothers, families, and health professionals in the community.

Key Responsibilities:

  • Provide education and counseling to new and young mothers to help reduce high-risk pregnancies.
  • Support families through home visits and act as a resource person for family needs.
  • Assist health professionals in providing adequate support to clients through translation or delivery of programs.
  • Promote and prevent illness to support clients in a holistic manner.

Requirements:

  • High School diploma.
  • Fluent in English and Cree, with French as an asset.
  • Demonstrated ability to translate information from Cree to a second language.
  • Motivation to work in a health field and promote healthy lifestyles.
  • Good critical thinking, synthesis, organizational, and decision-making skills.
  • Emotional maturity, autonomy, flexibility, discretion, and empathy.
  • Teamwork skills, dynamism, sense of accountability, and resourcefulness.
  • Good interpersonal and communication skills.
