We are seeking a skilled and compassionate Social Worker to join our team at FunctionAbility Rehabilitation Services.
Job Summary:The successful candidate will be responsible for conducting psychosocial assessments, evaluating the effectiveness of social programs, and providing mediation services to clients. They will also be required to prepare detailed plans for clients returning home after a stay in a rehabilitation centre, provide counsel and therapy to help clients resolve social and personal problems, and refer clients to agencies that provide financial assistance, legal aid, housing, medical treatment, and other services.
Key Responsibilities:- Conduct psychosocial assessments and evaluations to determine client needs and develop plans for assistance.
- Provide mediation services to resolve conflicts and facilitate communication between clients and other stakeholders.
- Prepare detailed plans for clients returning home after a stay in a rehabilitation centre, including recommendations for ongoing support and services.
- Provide counsel and therapy to help clients resolve social and personal problems, including substance abuse, mental health issues, and relationship conflicts.
- Refer clients to agencies that provide financial assistance, legal aid, housing, medical treatment, and other services.
- Collaborate with other professionals, including healthcare providers, social workers, and community organizations, to provide comprehensive support to clients.
- Develop and implement prevention and intervention programs to meet community needs and promote social change.
- Supervise professional staff, program leaders, and instructors to ensure high-quality services and support.
- Bachelor's degree in Social Work or a related field.
- 1 year to less than 2 years of experience in social work or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
- Strong assessment and evaluation skills, with the ability to develop and implement effective plans for assistance.
- Ability to work independently and as part of a team, with a strong commitment to collaboration and mutual support.
- Proficiency in English, with the ability to communicate effectively with clients and other stakeholders.
This is a permanent, on-the-road job with a 30-40 hour workweek. The successful candidate will be required to work in a variety of settings, including community organizations, rehabilitation centres, and clients' homes.
