Indigenous Patient Navigator Role

Vancouver Coastal Health is seeking a dedicated and compassionate Indigenous Patient Navigator to join our team. As a key member of our Indigenous Health Patient Experience team, you will play a vital role in ensuring the health and well-being of Indigenous clients and their families.

Key Responsibilities
  • Provide day-to-day health system navigation support to create safe and respectful environments for Indigenous clients and their families.
  • Liaise with clients and primary care teams to ensure successful access to primary care and community-based health care services.
  • Ensure patient care occurs in a culturally safe and respectful manner by connecting with clients and families, offering support, and providing traditional culturally safe resources.
  • Liaise with Indigenous community-based agencies and inform program development, planning, and evaluation of services for Indigenous clients.
Qualifications
  • Two to three years' recent experience working with Indigenous communities in British Columbia, with a combination of training, education, and/or experience in a similar field.
  • Bachelor's Degree in healthcare, social work, nursing, or a related discipline preferred.
  • Preference given to Indigenous applicants, including First Nations, Metis, and Inuit Peoples.
