We are seeking a dedicated Registered Nurse to become part of our Primary Care Team.
This role involves collaborating with healthcare providers and interdisciplinary teams to deliver comprehensive nursing care to patients and their families.
Key Responsibilities:
Work alongside the Primary Care Provider to assess, plan, implement, and evaluate nursing interventions for patients and their caregivers. Foster partnerships with healthcare professionals in line with the local care delivery model. Utilize health promotion and chronic disease management strategies to conduct thorough assessments and address patient care needs. Ensure continuity of care by coordinating with team members and facilitating transitions in care. Serve as a nursing resource by applying critical thinking and problem-solving skills in clinical practice.About the Community

Qualifications:
Education & Experience
Active registration as a Registered Nurse with the appropriate regulatory body. A minimum of one year of relevant clinical experience or an equivalent combination of education and training. A valid driver's license and access to a vehicle are required.Skills & Competencies:
Strong critical thinking abilities within the scope of patient care. In-depth knowledge of primary care practices and patient-centered care models. Familiarity with professional regulations and standards relevant to community health nursing. Competence in evidence-based clinical practices related to primary care. Understanding of health management principles across various health conditions. Ability to provide culturally safe care to diverse populations. Effective communication and conflict resolution skills. Capability to work independently and prioritize patient care effectively. Proficiency in utilizing technology for patient care documentation.
