As a key member of our team, you will provide administrative and clerical support to our Care Coordination staff, ensuring that patients receive prompt and effective customer service.

Key Responsibilities:
  • Provide information and referral services to patients, families, service providers, and the general public.
  • Communicate with team members to ensure information is distributed and obtained appropriately.
  • Maintain accurate and up-to-date client records, including scanning and data entry.
  • Process requisitions, prepare related documents, and monitor inventory of supplies.
  • Arrange client transportation and book appointments.
  • Assist with projects and initiatives related to job responsibilities.
Requirements:
  • High school graduation diploma plus one year of Community College diploma in Office Administration, Business, Health Administration, or equivalent program.
  • Minimum 18 months of experience in customer service or related position in the health, social, or business administration fields.
  • Proficiency in computer software applications, including email and the internet.
  • Strong oral and written communication skills.
  • Ability to work independently and as part of a team.
