Job Summary:

The Community Service Worker will provide support and services to individuals and families in need, focusing on education, experience, and skill development. The successful candidate will work collaboratively with clients, community agencies, and healthcare providers to deliver effective services and promote positive outcomes.

Key Responsibilities:
  • Prepare and maintain accurate records and reports of client interactions and progress.
  • Assess client needs and develop personalized plans to address their goals and objectives.
  • Implement life skills workshops and training programs to enhance client independence and self-sufficiency.
  • Conduct regular assessments and evaluations to monitor client progress and adjust service plans as needed.
  • Liaise with community agencies and healthcare providers to ensure seamless service delivery and coordination.
  • Provide crisis intervention and support services to clients in need.
  • Supervise and mentor clients in group homes and half-way houses.
  • Develop and implement substance abuse treatment programs and behavior management plans.
  • Conduct individual and group counseling sessions to promote client well-being and empowerment.
  • Maintain accurate program statistics and contribute to evaluation and research efforts.
Requirements:
  • Secondary (high) school graduation certificate.
  • 2 years to less than 3 years of experience in a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and crisis management skills.
  • Knowledge of community resources and services.
Work Environment:

The Community Service Worker will work in a fast-paced, dynamic environment with a diverse client population. The successful candidate will be required to work flexible hours, including evenings and weekends, as needed.
