Pro Vita Care Management Inc. is seeking a dedicated Lead Licensed Practical Nurse who is passionate about providing exceptional care to the elderly population. This role is essential in supporting our residents within the Long Term Care sector.
Position Overview:
The Lead Licensed Practical Nurse will play a pivotal role in overseeing the nursing department, ensuring that all care provided aligns with the organization's mission and values.
Key Responsibilities:
- Deliver strong leadership within the nursing team, adhering to the organization's mission, vision, and values.
- Design and implement training programs for nursing staff.
- Create and monitor individual performance plans to align with strategic objectives.
- Evaluate nursing practices to ensure compliance with professional standards and regulations.
- Maintain high-level internal and external relationships to enhance care delivery.
- Collaborate with healthcare professionals to ensure comprehensive resident care.
- Conduct medication reconciliation and oversee admission and discharge audits.
- Engage in discharge planning and coordinate transportation for residents as needed.
Qualifications:
- Completion of a recognized Practical Nursing program or equivalent education and experience.
- Active registration with the relevant nursing regulatory body.
- Valid Occupational First Aid certification and CPR certification.
- Minimum of two years of experience in Gerontological or Assisted Living settings.
- Familiarity with RAI MDS education (Gold Care or Point Click Care).
- Ability to meet the physical demands of the position.
