Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of residents' physical and emotional needs, develop and implement individualized care plans, and collaborate with the interdisciplinary team to ensure seamless care delivery.
- Medication Management: Administer medications and treatments as prescribed by physicians, monitor residents' responses to medications, and report any adverse reactions.
- Wound Care and Management: Identify and manage wounds, pressure ulcers, and other skin conditions, providing appropriate treatment and interventions to promote healing and prevent further complications.
- Communication and Collaboration: Communicate effectively with residents, families, and healthcare team members to ensure comprehensive care, address concerns, and provide education on health-related topics.
- Documentation and Reporting: Maintain accurate and detailed records of resident care, including assessments, care plans, and progress notes, and report any changes or concerns to the healthcare team.
- Education: Current registration with the College of Nurses of Ontario (CNO) and a diploma or degree in nursing from an accredited program.
- Experience: Minimum 3 years of experience in long-term care or a related setting, with a strong focus on gerontology and wound care.
- Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and strong attention to detail.
