Key Responsibilities- Provide Patient-Focused Care: Deliver safe, holistic nursing care to patients, assessing their needs and responding to their changing requirements.
- Collaborate with Healthcare Teams: Work effectively with other healthcare professionals to ensure seamless care delivery and achieve optimal patient outcomes.
- Travel and Mobility: Travel within your designated region to provide care to patients in their homes.
- Minimum 2 Years of Nursing Experience: Possess a minimum of two years of experience as a Registered Practical Nurse, with a preference for community nursing experience.
- Registered with the College of Nurses of Ontario: Be registered and in good standing with the College of Nurses of Ontario (CNO).
- Ability to Work Independently: Demonstrate the ability to work independently in the community, with minimal supervision.
- Language Proficiency: Possess proficiency in English, both verbal and written, with additional languages being an asset.
- Valid Ontario Driver's License: Hold a valid Ontario driver's license and have access to a reliable vehicle.
- Specialized Skills: Possess experience with infusion therapy, mental health, palliative care, and wound care, as well as excellent patient assessment and teaching skills.
