ResponsibilitiesHealth and Wellness Programs- Co-facilitate in-person and community-based programs focused on life-skill development, capacity building, volunteerism, and social networking.
- Pick up program items and complete budget/expense forms.
- Support program recruitment, registration, setup, and community displays.
- Assist with program planning, evaluation, data entry, and reports.
- Support Health and Wellness team at community events, front desk, and other duties.
- Promote and distribute Healthy Smiles Ontario information through direct contact with community members and partners.
- Compile and submit dental program data to funders.
- Promote and distribute Ontario Seniors Dental Care Program information.
- Facilitate oral health workshops, education groups, and community health fairs targeting children, youth, parents, and seniors.
- Identify eligible clients, assist with eligibility forms, and share oral health awareness resources.
- Participate in team and staff meetings, and other meetings as required.
- Work from a Social Determinants of Health perspective to improve health outcomes and reduce health inequities.
- Maintain confidentiality, client safety, and minimize risk to clients, volunteers, and the Centre.
- Work in accordance with the Health Centre's Vision, Mission, and Values.
- Adhere to all Health Centre policies and procedures.
- Perform other duties as assigned by your supervisor.
