Job Summary

The Personal Support Worker provides personalized support and homemaking services to clients and families within the framework of VON Canada's policies, procedures, and standards. This role requires collaboration with the healthcare team to meet clients' needs and help them achieve an optimal level of independence and well-being.

Key Responsibilities
  • Assess and evaluate clients' or families' homes, personal situations, and care plans to determine if any changes are required.
  • Contribute to the development of personalized care plans under the direction of the Personal Support Program Manager.
  • Implement changes to care plans under the guidance of the Manager for safety, liability, and financial reasons.
  • Report verbally to the immediate supervisor or Personal Support Program Manager any changes in clients' or families' personal support needs or home situations.
  • Perform personal support tasks reflecting clients' and families' needs, including light cleaning, meal preparation, care of clothes, shopping, and monitoring household safety.
  • Provide personal care as directed by the Personal Support Program Manager or immediate supervisor, including assistance with walking, climbing stairs, getting in and out of bed, eating, dressing, and personal hygiene.
  • Provide simple bedside care under the direction of a nurse.
  • Offer respite care to clients as required.
  • Support clients' families by providing childcare as needed to allow for family relief.
  • Organize daily activities, setting priorities for personal support and other tasks under the direction of the Personal Support Program Manager or immediate supervisor.
  • Complete daily call checks and other forms as required to maintain the effective and efficient operation of the personal and home support service.
  • Assess clients' patterns of communication and ability to relate.
  • Communicate with healthcare team members regarding clients' homemaking, physical, and emotional needs and inform the immediate supervisor or Manager of any changes or concerns.
Requirements
  • High school diploma.
  • Secondary school education is preferred.
  • CCA Certification is required in Nova Scotia.
  • Must have a certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
  • Must have a current Standard First Aid Certificate or CPR level C Certificate, if applicable to the work being performed.
  • Some household management experience and experience working with elderly or ill individuals or children.
Skills
  • Strong communication skills, both verbally and written.
  • Able to communicate effectively with clients and families in a clear and effective manner that demonstrates respect, empathy, and understanding.
  • Good assessment skills.
  • Awareness of community resources.
  • Ability to work effectively in a multidisciplinary team or independently as required.
  • Able to take direction well.
  • Ability to adjust to working in different social and economic environments.
  • Capacity to adhere to strict confidentiality.
