Key Responsibilities- Conduct social assessments and provide practical support to clients and families
- Assist clients with navigating healthcare systems and accessing community resources
- Maintain accurate records and participate in interdisciplinary clinical conferences
- Develop and implement individualized care plans to meet the unique needs of clients
- Current full registration with the British Columbia College of Social Workers
- Bachelor's degree in Social Work from an approved school of Social Work
- Two years' recent experience working with marginalized clients with multiple health issues
- Valid BC Driver's License and access to a personal vehicle for business-related purposes
- Basic knowledge of systems thinking and social work process
- Working knowledge of counselling skills and trauma-informed practice
- Understanding of local initiatives and resource access in the community
- Demonstrated ability to promote Indigenous Cultural Safety and provide culturally safe care
