About the Role:The Assessor Coordinator will report directly to the Manager of Children's Therapies and be responsible for assessment/reassessment services with ongoing client care planning and management. The successful candidate will establish and maintain positive liaison between clients, referral sources, and Children's Services, ensuring safe care for individuals.
Key Responsibilities:- Screening, referring, reviewing, planning, and facilitating safe care for individuals
- Establishing and maintaining positive liaison between clients, referral sources, and Children's Services
- Providing assessment/reassessment services with ongoing client care planning and management
- One (1) year related experience in healthcare or social sciences
- Bachelor of Science in Nursing (BScN) or equivalent
- Registered/eligible for registration with the appropriate licensing body
- HSAS membership
- Community experience
