We are seeking a dedicated Registered Nurse to contribute to our Primary Care Team.
This role involves working collaboratively within an interdisciplinary team to provide comprehensive nursing care for patients and their families.
Key Responsibilities:
Work alongside the Primary Care Provider and other healthcare professionals to assess, plan, implement, and evaluate nursing care. Maintain a collaborative relationship with the Primary Care Provider, adhering to the established care delivery model. Utilize health promotion and chronic disease management strategies to conduct assessments and provide direct patient care. Ensure continuity of care by coordinating with team members and supporting transitions between services. Serve as a nursing resource by applying critical thinking and problem-solving skills to address patient needs.Community Insights

Qualifications
Education & Experience
Current registration as a Registered Nurse with the appropriate regulatory body. Minimum of one year of relevant clinical experience or an equivalent combination of education and training. Valid driver's license and access to transportation.Skills & Competencies
Proven ability to apply critical thinking within the scope of patient care. Strong understanding of primary care principles and patient-centered care models. Familiarity with professional regulations and standards relevant to community health nursing. Knowledge of evidence-based practices in primary care. Understanding of provincial regulations and policies related to community health. Ability to provide culturally safe care across diverse populations. Competence in promoting Indigenous Cultural Safety. Familiarity with Trauma Informed Practice and Harm Reduction principles. Effective communication and conflict resolution skills. Capacity to work independently and prioritize patient care effectively. Proficiency in using computerized patient care documentation systems.
