Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Fraser Health. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to a positive and supportive work environment.

Key Responsibilities
  • Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
  • Provide direct patient care, including administering medications, performing treatments, and monitoring patient progress.
  • Contribute to the development and implementation of unit-specific protocols and policies.
  • Participate in multidisciplinary care planning meetings and family conferences to ensure comprehensive patient care.
  • Document patient information and maintain accurate records to ensure continuity of care.
  • Collaborate with other healthcare professionals to ensure seamless patient care and promote a culture of safety and quality.
Requirements
  • Graduate of a recognized practical nursing program or equivalent combination of education and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives.
  • Ability to communicate effectively and work collaboratively as part of a healthcare team.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
