Key Responsibilities- Develop and maintain effective care plans in collaboration with the Ontario Health atHome team.
- Provide nursing services to patients in various settings, including in-home, clinics, and retirement homes.
- Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care.
- Perform other clinical duties as required.

- PAID MILEAGE at $0.45 per/km PLUS travel time.

- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.

- Exceptional interpersonal skills.
- A valid driver's license and vehicle.
- Excellent English verbal and written communication skills.
- CPR Certification.
- Current immunization record including 2 step TB test.
- To protect our patients, we require a current vulnerable sector check and a clear background check.
