Job Summary

The Clinical Social Worker Associate is a key member of our multidisciplinary treatment team, responsible for providing high-quality clinical services to patients and their families. This role requires a strong commitment to delivering exceptional patient care, while also collaborating with internal and external stakeholders to ensure seamless transitions and effective discharge planning.

Key Responsibilities
  • Provide ongoing assessment and formulation of patient needs, updating treatment plans to meet clinical and regulatory requirements.
  • Deliver clinical services to patients and family members, as indicated, and coordinate the delivery of services by the patient's multidisciplinary treatment team.
  • Act as primary liaison with family and significant others, referrers, and community agencies and professionals to develop a timely and comprehensive discharge plan.
  • Collaborate with the Utilization Manager to manage patient care in a clinically and fiscally sound manner, and serve as designated contact with managed care organizations.
  • Ensure timely completion of all documentation and other required paperwork needed to facilitate patient evaluation, transition, and billing.
Qualifications
  • Master's degree in Social Work (MSW)
  • Attaining Massachusetts LCSW licensure is not required upon hire, but is a requirement for continued employment within 12 months of hire or upon the one year anniversary of employment.
  • Exceptional clinical skills, including psychiatric assessment, crisis stabilization, and family support and intervention.
  • Excellent written and verbal communication skills, with ability to work collaboratively within a rapidly changing system of care.
  • Familiarity with computerized clinical data bases and electronic medical records.
