The Clinical Social Worker Associate is a key member of our multidisciplinary treatment team, responsible for providing comprehensive care to patients and their families. This role requires a strong clinical foundation, excellent communication skills, and the ability to work collaboratively with internal and external stakeholders.
Key Responsibilities- Provide ongoing assessment and formulation of patient needs to update treatment plans.
- Coordinate delivery of services by the patient's multidisciplinary treatment team.
- Act as primary liaison with family and significant others, referrers, and community agencies to develop timely and comprehensive discharge plans.
- Collaborate with the Utilization Manager to manage patient care in a clinically and fiscally sound manner.
- Ensure timely completion of all documentation and other required paperwork.
- Master's degree in Social Work (MSW).
- Attaining Massachusetts LCSW licensure within 12 months of hire.
- Excellent written and verbal communication skills.
- Ability to work collaboratively within a rapidly changing system of care.
- Experience in providing a range of clinical services, including assessment, treatment, and discharge planning.
