St.Amant is seeking a dedicated and compassionate Personal Support Worker - Healthcare (PSW-H) to join our team

In this role, you will provide exceptional care and support to individuals in our residential programs, empowering them to live fulfilling lives.

Responsibilities:
  • Direct Support: Assist individuals with daily living activities such as personal hygiene, dressing, meal preparation, and medication administration.
  • Health Monitoring: Observe and document changes in an individual's health status, reporting any concerns to the appropriate staff.
  • Safety & Well-being: Ensure a safe and supportive environment for individuals, implementing safety protocols and responding to emergencies effectively.
  • Individualized Care Plans: Contribute to the development and implementation of individualized care plans, ensuring they reflect the unique needs and goals of each person.
  • Communication & Documentation: Maintain accurate and timely records of individual's care, progress, and any significant events. Communicate effectively with families, colleagues, and other healthcare professionals.
Quality Management & Process Improvement:

The PSW-H actively participates in continuous quality improvement initiatives:

  • Collects and records data related to individual or program goals.
  • Recommends opportunities for improvement and collaborates on project teams as assigned.
Organizational Planning & Development:

The PSW-H supports the achievement of service area, program, and organizational objectives:

  • Participates in strategic planning processes at both organizational and service levels.
  • Actively participates in team meetings and committees as assigned.
  • Engages in ongoing professional development through approved training, seminars, and educational conferences.
