Job Summary

The Chronic Disease Education Nurse is an integral member of a collaborative interdisciplinary primary health care team at Prairie Mountain Health. This role is responsible for providing prevention, education, care, and support services for clients of the Diabetes and Heart Health Program.

Key Responsibilities
  • Client Assessment and Care Planning: Conduct comprehensive assessments of clients' needs, develop and implement individualized care plans, and evaluate outcomes to ensure effective delivery of client care services.
  • Chronic Disease Self-Management Education and Support: Provide education and support to clients to empower them to manage their chronic diseases effectively, including individual and group sessions.
  • Prevention Activities: Implement prevention activities in the community using the population health promotion model, focusing on health promotion, health education, motivational interviewing, behavior change, and harm reduction strategies.
  • Interdisciplinary Collaboration: Work collaboratively with other healthcare professionals to provide comprehensive care to clients, including liaison, consultation, routine and priority reporting, and referral as required.
  • Administrative Responsibilities: Complete documentation according to nursing standards and PMH regional and program policies and guidelines, including reports to other healthcare providers as appropriate.
  • Research and Evaluation: Participate in research activities and utilize results in evidence-informed practice to improve client care and services.
Requirements
  • Current registration with the College of Registered Nurses of Manitoba.
  • Experience in chronic disease management and education.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Strong analytical and problem-solving skills.
  • Ability to prioritize workload effectively.
