About the Opportunity

We are seeking a skilled Medical Social Worker to join our team at Nova Scotia Health Authority. As a Medical Social Worker, you will provide professional social work services to patients and their families, including support groups and home visits.

About You

We are looking for a candidate with a Master's Degree in Social Work, or a Bachelor of Social Work with relevant experience. You should be registered or eligible for registration with the Nova Scotia College of Social Workers and have a minimum of two years of clinical experience. You should also have extensive knowledge of community resources and be able to work independently with a multidisciplinary team.

Key Responsibilities
  • Provide professional social work services to patients and their families, including support groups and home visits.
  • Develop and implement care plans for hospital discharge and safe transition back to the community or institutional setting.
  • Coordinate and document patient services, including referrals to community resources.
  • Work collaboratively with a multidisciplinary team to provide patient-centered care.
  • Travel between sites as required.
Requirements
  • Master's Degree in Social Work or Bachelor of Social Work with relevant experience.
  • Registered or eligible for registration with the Nova Scotia College of Social Workers.
  • Minimum of two years of clinical experience.
  • Extensive knowledge of community resources.
  • Ability to work independently with a multidisciplinary team.
  • Competencies in other languages an asset.
