We are seeking a dedicated and compassionate Community Worker to join our team at Independent Living Nova Scotia Association. As a Community Worker, you will play a vital role in supporting individuals with various needs, promoting independence, and fostering a sense of community.
Job Summary:The successful candidate will be responsible for providing support and guidance to clients, assisting with daily living tasks, and connecting them with community resources. You will work collaboratively with our team to develop and implement personalized plans, ensuring the best possible outcomes for our clients.
Key Responsibilities:- Provide emotional support and guidance to clients, promoting independence and self-advocacy.
- Assist clients with daily living tasks, such as meal preparation, medication management, and personal care.
- Connect clients with community resources, services, and support groups.
- Develop and implement personalized plans, working collaboratively with the client and our team.
- Monitor and report on client progress, making adjustments as needed.
- Participate in team meetings and contribute to the development of new programs and services.
- Secondary (high) school graduation certificate.
- Experience working with individuals with various needs, preferably in a community setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility and adaptability in a fast-paced environment.
