As a Cancer Nurse Navigator, you will play a vital role in enhancing access to cancer support services for clients, families, and external providers. Your key responsibilities will include:
Key Responsibilities:- Acting as a single point of contact during key transitions along the cancer care continuum, including initial diagnosis, active treatment, and end-of-treatment phases.
- Collaborating with Territorial, Regional, and Community Health Centre sites, as well as visiting specialists and facilities outside the NWT, to provide comprehensive and integrated care.
- Engaging in the coordination of safe, integrated, and individualized care across the cancer care continuum, and providing education on survivor care plans.
- Promoting a holistic approach to planning, organizing, teaching, and relationship development that meets the needs of clients and their families.
- Acting as an advocate, facilitating communication between clients, their families, Family Practitioners, specialists, and other healthcare professionals.
- A Baccalaureate Degree in Nursing and completion or willingness to complete an oncology nursing program.
- Minimum of 5 years nursing experience, with a minimum of 2 years of oncology nursing experience preferred.
As a Cancer Nurse Navigator, you will be an integral part of the interdisciplinary team, working collaboratively to provide exceptional care to clients and their families.
