Role Overview:
The Transitional Support Specialist will utilize engaging activities to assist individuals living with dementia in maintaining their independence at home and in social settings. Through interactive games, creative arts, and recreational activities, they will help families support the physical and emotional health of those living with dementia, facilitating their integration into community life.
The Dementia Care Navigator will play a crucial role in coordinating and integrating various support services tailored to the needs of individuals living with dementia and their caregivers. Acting as the primary point of contact for families post-diagnosis, this role involves assessing needs, fostering self-management, and enhancing communication and care planning among healthcare providers across different sectors. The Dementia Care Navigator aims to ensure that every individual diagnosed with dementia and their caregivers have timely access to essential information, educational resources, and support, striving to achieve the following objectives:
- Expand the capacity of the system to provide navigation support for families facing dementia challenges.
- Enhance the overall experience and health outcomes for individuals living with dementia and their caregivers.
- Empower caregivers with the skills and knowledge necessary to manage their responsibilities effectively and reduce the likelihood of crisis situations.
- Increase the ability of individuals living with dementia to remain in their homes and communities for as long as possible.
Qualifications:
Educational Requirements:
- For the Care Navigator role - Minimum Social Service Worker Diploma.
- For the Transitional Support Specialist role - Minimum Personal Support Worker Certificate.
- The combined role requires a Registered Practical Nurse diploma.
Experience:
- 1 to 3 years of experience in client service within health or social services.
- Direct experience working with individuals living with Alzheimer's disease or other forms of dementia and their caregivers.
- Knowledge in managing chronic and complex health conditions.
- Familiarity with community resources and various care options.
- Understanding of the interconnections between primary care, community services, and specialized geriatric care.
- Strong foundation in client-centered care principles.
- Knowledge of dementia-related training models and clinical practices.
- Experience in care assessment and planning.
- Collaboration experience in interdisciplinary settings.
- Above-average presentation and adult education skills.
- Proven ability to work effectively in a remote team environment.
- Experience in facilitating support groups and educational sessions in diverse formats.

- Proficient in developing and delivering educational content to large audiences.
- Excellent verbal and written communication skills.
- Strong interpersonal abilities, including shared decision-making and facilitation skills.

Travel Requirements:
Some travel within the district is necessary, with a minimum weekly commitment to the designated office location.
Physical Demands:
This role does not require any special physical demands beyond standard office duties. The position involves extended periods of sitting at a computer, with opportunities for movement. Sensory attention is essential for most of the workday, including screen time and document review.
