Job Summary

We are seeking a skilled Registered Nurse to join our team as a SNF Float RN. In this role, you will provide comprehensive patient care in an independent setting, working autonomously to assess, diagnose, and implement complete care plans. You will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, troubleshooting and making sound decisions for our patients related to their dialysis treatment.

Responsibilities
  • Provide direct patient care, supervising Dialysis Technicians as needed
  • Conduct impactful and meaningful education to patients and their families in the SNF setting
  • Work collaboratively with SNF staff to coordinate care and ensure seamless transitions
  • Meet and/or exceed clinical outcomes, multitasking to meet the needs of the service, patients, and SNF partners
Requirements
  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months' of RN experience
  • Previous Chronic Hemodialysis, Acute Dialysis, and/or Peritoneal Dialysis (PD) nursing experience
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
  • Basic computer skills and proficiency in MS Word and Outlook
Preferred Qualifications
  • Training/Teaching/Healthcare Education background
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)
