Flemingdon Health Centre is seeking a skilled Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our multidisciplinary team to deliver exceptional patient outcomes.
Key Responsibilities- Provide comprehensive primary care to patients, including health assessments, diagnosis, and treatment of acute and chronic conditions.
- Develop and implement individualized care plans to meet the unique needs of patients.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between healthcare settings.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Contribute to the development and implementation of policies and procedures to ensure compliance with regulatory requirements and organizational standards.
- Current registration with the College of Nurses of Ontario (CNO).
- Master's degree in Nursing or related field.
- Minimum 2 years of experience as a Nurse Practitioner in a primary care setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Competitive salary range: $53,928 - $64,828 per year (pro-rated for 17.5 hours/week).
