Enhanced Home Support Specialist

About the Role
CarePartners is seeking a dedicated and compassionate Enhanced Personal Support Worker to join our team. As a key member of our eHomeCare program, you will provide high-quality care and support to patients in their own homes, working collaboratively with our specially trained nurses.
Key Responsibilities
- Deliver care and support to patients and their families in their home
- Provide quality, integrated care through direct communication between a nurse via laptop/tablet and the eHomeCare Support Worker via smartphone throughout their shift

Requirements
- A strong desire and commitment to making a difference in the lives of their patients
- Passion for providing holistic, sensitive, individualized care to patients at end of life
- Experience with providing care to patients in their own homes (considered an asset)
- Ability to work independently and take direction and feedback effectively
- Technological skills, such as smartphones and virtual care delivery platforms
- Exceptional interpersonal skills
- Ability to work both independently and as part of a team
- Availability to work non-traditional hours, such as 12-hour night shifts and alternate weekends
- A valid driver's license and vehicle
- Valid certificate as a Personal Support Worker or Internationally Trained Nurse or enrollment in a Nursing program
- To protect our patients, we require current vulnerable sector check screening, and a clear background check
Additional Incentives
- New Ontario PSW grads may be eligible to receive a $10,000 bonus, payable at 6 months and 12 months, in exchange for a commitment to work with us in a Full-Time position for 12 months
- An additional $10,000 relocation bonus, payable at start and 6 months, when relocating to rural, remote, or northern areas and committing to working with us for 12 months
