Vancouver Coastal Health is seeking a skilled Social Worker to join our Long Term Care team in Powell River, BC. As a Social Worker with our organization, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.
Key Responsibilities- Provide advanced clinical and consultative social work services to clients and families, focusing on how the client's relationships are affected by their environment.
- Conduct comprehensive social and psycho-social assessments to identify client needs and develop effective intervention strategies.
- Provide individual, family, and group counseling services to support clients in achieving their goals and improving their overall well-being.
- Maintain accurate and up-to-date client records, ensuring confidentiality and adherence to regulatory requirements.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that address the unique needs of each client.
- Identify and connect clients with community resources and services to support their ongoing care and well-being.

Qualifications- Current full registration with the British Columbia College of Social Workers (BCCSW).
- Master of Social Work from an accredited School of Social Work and one (1) year post-Bachelor of Social Work experience in a healthcare setting.
- Broad knowledge of systems thinking, other health disciplines, research methodology, counseling skills, and clinical practice models.
- Demonstrated ability to apply systems theory to practice in social work assessment and intervention, conduct social and psycho-social assessments, and establish rapport with clients and families.
- Effective communication and interpersonal skills, with the ability to work collaboratively as a member of an interdisciplinary team.
