Clinical Pharmacist Role Overview
The Clinical Pharmacist will play a key role in providing individualized drug therapy to patients with cancer, utilizing patient information to develop therapeutic plans. This will involve collaborating with other healthcare professionals to ensure appropriate medication therapy and developing discharge plans for inpatients and medication calendars for outpatients.
Key Responsibilities
- Provide individualized drug therapy to patients through methods such as receiving patient charts, assessing clinical response, and creating individualized therapeutic plans.
- Interact with patients and the healthcare team to ensure appropriate medication therapy and develop therapeutic plans through methods such as developing discharge plans and medication calendars.
- Prepare and dispense medications and other pharmaceutical supplies to ensure appropriate medication therapy and adherence to relevant federal and provincial legislation.
- Provide verbal and written drug information to physicians, nurses, and other healthcare professionals within the Agency, as well as to patients through individualized counseling.
- Assist with the provision and/or financial support of non-BCCA benefit medications required to optimize the therapeutic plan.
- Provide instruction and/or supervision of pharmacy students and/or pharmacy residents through methods such as mentoring.
Requirements
- Bachelor's Degree in Pharmacy with current registration as a Licensed Pharmacist with the College of Pharmacists of British Columbia.
- Completion of a Hospital Pharmacy Residency Program.
- One (1) year of recent related experience or an equivalent combination of education, training, and experience.
