Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients to identify their needs and develop individualized care plans.
- Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring patient safety and comfort.
- Wound Care and Management: Provide wound care and management services, including dressing changes and wound assessments.
- Monitoring and Reporting: Monitor patients' vital signs and report any changes or concerns to the healthcare team.
- Education and Support: Educate patients and their families on disease management, medication use, and other health-related topics.
- Licensure and Certification: Current licensure as a Licensed Practical Nurse (LPN) in the province.
- Education and Training: Completion of a recognized LPN program and ongoing education and training in areas such as wound care and medication administration.
- Experience and Skills: Minimum 1 year of experience as an LPN in a long-term care or skilled nursing setting, with strong communication and interpersonal skills.
