About Spectrum Health Care
We are a leading home healthcare provider in Canada, committed to delivering exceptional patient care and support. As a Client Care Coordinator, you will play a vital role in our team, working collaboratively with community PSWs and Nurses to provide seamless care services.
Key Responsibilities
- Work closely with community PSWs and Nurses to schedule and coordinate services
- Liaise with clients, staff, and community partners to ensure effective communication and care delivery
- Act as a problem solver for clients, responding to inquiries from LHIN and employees
- Report client and staff concerns to the Home Support Supervisor
- Liaise with LHIN representatives regarding client care
- Collect and enter client data into our computer system
- Provide administrative support to the personal support worker and Nurse program
Requirements
- Minimum 2 years of experience in an office setting
- Excellent English written and verbal communication skills
- Ability to handle high volumes of calls while delivering exceptional customer service
- Ability to multitask and prioritize
- High school diploma or equivalent
- Proficient in Word, Outlook, and ability to learn proprietary software
