Key Responsibilities- Provide patient-centered care to individuals with complex needs, working collaboratively with the healthcare team to develop and implement effective care plans.
- Assess and manage patient conditions, administering medications and performing physical assessments as required.
- Develop and maintain strong relationships with patients and their families, providing education and support to promote optimal health outcomes.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in patient care.
- Current registration with the College of Nurses of Ontario.
- Current Heart & Stroke Approved BLS Provider Card upon hire.
- Experience working with and advocating for the elderly, with a strong focus on patient-centered care.
- Medication administration certification and experience, with a strong understanding of wound care and physical assessment.
- Experience with electronic documentation and maintenance of IV lines.
- Satisfactory employment and attendance record.
- Experience with enteral feeding and maintenance of IV lines.
- Strong communication and interpersonal skills, with the ability to work effectively in a dynamic and rapidly changing environment.
- Ability to foster teamwork and cooperation, with a strong focus on patient-centered care.
