Job SummaryWe are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Visiting Nurse. As a Visiting Nurse, you will provide exceptional nursing services to patients in their homes, working independently or in cooperation with the Home and Community Care Support Services (HCCSS).
Key Responsibilities- Develop and maintain effective care plans for patients, taking into account their unique needs and circumstances.
- Provide one-on-one nursing services to patients in their homes, including assessments, treatments, and education.
- Communicate effectively with patients, family members, and other healthcare professionals to ensure seamless care coordination.
- Perform other clinical duties as required, including administering medications, wound care, and vital sign monitoring.
- Current College of Nurses of Ontario (CNO) registration in good standing.
- Strong interpersonal skills and a commitment to delivering exceptional patient care.
- A valid driver's license and vehicle, as regular travel is required.
- Excellent English verbal and written communication skills.
- CPR certification and current immunization record, including a 2-step TB test.
