Key Responsibilities

  • Provide one-on-one care to patients, including assistance with daily living activities, mobility, and personal care.
  • Collaborate with healthcare teams to develop and implement patient care plans that promote independence, dignity, and well-being.
  • Support patients with dementia or cognitive impairments, using evidence-based practices and person-centered care approaches.
  • Participate in multidisciplinary team meetings to share knowledge, expertise, and best practices.
  • Contribute to a culture of caregiver inclusion, promoting open communication, respect, and empathy among team members.

Requirements

  • Graduate of a Personal Support Worker Program from a recognized Community College.
  • Holds current certification in Basic Cardiac Life Support.
  • Demonstrated ability to work with patients with dementia or cognitive impairments.
  • Demonstrated knowledge and practice of good body mechanics.
  • Good listening skills and ability to understand, interpret, and carry out written or verbal instructions.
  • Ability to work as a member of the multidisciplinary team and demonstrate commitment to patient safety.
  • Proven ability to work with minimal supervision and demonstrate regular, punctual attendance.
  • Functional proficiency in English (oral and written) and limited proficiency in oral French.
