Position Overview:

The Nurse Practitioner (NP) at Tri-County Mennonite Homes operates under the supervision of the Executive Directors, providing a blend of clinical practice, consultation, education, and advocacy within our healthcare facilities. This role emphasizes autonomous practice, delivering comprehensive primary health care tailored to the unique needs of our residents.

Key Responsibilities:

  • Adhere to legislative and regulatory standards while fulfilling the responsibilities of the Nurse Practitioner role.
  • Practice within the legislated scope, including diagnosing, prescribing, and interpreting diagnostic tests, while delivering specialized care to residents with complex health needs.
  • Collaborate with interdisciplinary teams to ensure thorough assessment, treatment planning, and evaluation of resident care.
  • Consult with and refer to other healthcare professionals and community resources as necessary.
  • Provide medical coverage as requested by medical leadership.
  • Guide and support staff in navigating challenging clinical scenarios, contributing to the development of clinical policies and guidelines.
  • Educate staff, residents, and families, fostering engagement and autonomy while developing educational programs and resources.
  • Advocate for evidence-based healthcare practices, assisting residents and families with advanced care planning and treatment consent.
  • Participate in management meetings and relevant committees to support organizational goals.
  • Ensure safe work practices and compliance with occupational health and safety regulations.

Leadership Duties:

  • Demonstrate exceptional interpersonal skills to mentor and develop nursing staff.
  • Oversee recruitment, training, and ongoing clinical competency assessments.
  • Communicate effectively with team members to foster a collaborative environment.

Clinical Management:

  • Guide the development and approval of policies and procedures related to nursing practice.
  • Manage equipment maintenance and ensure adherence to budgetary guidelines.

Quality and Safety Management:

  • Implement quality improvement initiatives to enhance service delivery and minimize risks.
  • Analyze data to identify trends and coordinate corrective actions.

Qualifications:

  • Current registration as a Nurse Practitioner with the College of Nurses of Ontario.
  • Proven expertise in geriatric care and chronic disease management.
  • Strong communication, leadership, and problem-solving abilities.
  • Ability to adapt to a dynamic work environment while maintaining high standards of care.
