We are seeking a skilled Community Health Worker II to join our team at Northern Health Authority. As a key member of our interprofessional healthcare team, you will provide home support services to clients in accordance with a pre-established care plan.
Key Responsibilities- Assist clients with activities of daily living, such as bathing, grooming, and oral hygiene
- Plan and prepare meals while promoting maximum independence
- Perform delegated tasks for which delegation of tasks training has been completed
- Ensure the well-being of individuals and families and promote independent living while in the community
- Comprehensive benefit packages, including MSP, extended health/dental, and a municipal pension plan
- Three weeks vacation with one year of continuous service
