Position Overview:
The Rapid Response Network (RRN) offers essential services to individuals transitioning from hospital to home, including in-home visits for chronic and acute illness management, Remote Care Monitoring, and pandemic response initiatives. By providing health education, symptom management, and medication reconciliation, the program aims to minimize hospital and emergency room visits, ensuring patients receive the necessary support in their communities.
The Nurse Practitioner serves as a key resource for multidisciplinary healthcare teams, collaborating with the Manager of Home and Community Care to deliver primary care that meets the needs of at-risk patients while fulfilling the objectives of the RRN program.
Key Responsibilities:
• Provide comprehensive primary healthcare services, including medication reconciliations, prescribing, lab orders, chart reviews, and physical assessments for at-risk patients facing healthcare access challenges.
• Collaborate with existing patient physicians and community partners to address patient needs and goals.
• Deliver health promotion and education tailored to the unique needs of patients in the RRN Program.
• Educate patients on effective disease prevention strategies and the maintenance of health.
• Conduct targeted assessments based on escalations from RRN or Care Coordinators and provide actionable recommendations.
• Offer consultation and support to the RRN Program and Home and Community Care Support Services Care Coordinators.
• Contribute to best practices by proposing ideas and opportunities for discussion with RRN Program leadership.
• Assist in the collaborative development and implementation of RRN Program policies and procedures.
• Advocate for the program as needed, ensuring positive public relations and effective service coordination through active participation in internal and external committees.
• Engage in ongoing professional development activities, including academic rounds and educational events.
Qualifications and Experience:
• Current registration with the College of Nurses of Ontario as a Registered Nurse (Extended Class) and eligibility to practice as a Nurse Practitioner.
• A minimum of three years of relevant nursing experience in a home or community care setting, including connections with community health agencies and other healthcare disciplines.
• Experience in case management and discharge planning is preferred.
Knowledge Requirements:
Subject Matter Expertise:
• Advanced assessment and diagnostic reasoning capabilities.
• Proven knowledge in consultation and ethical decision-making.
Champlain HCCSS Knowledge:
• Familiarity with Champlain HCCSS operations, quality, risk, and performance management principles.
• Understanding of relevant healthcare legislation.
• Knowledge of Champlain HCCSS policies, procedures, strategic direction, and objectives.
• Awareness of Champlain HCCSS partners and contracted service providers within the community.
General:
• Ability to accurately complete necessary documentation, reports, and recommendations.
• Proficient in computer software, including email, internet, and Microsoft Office applications (Word, Excel, PowerPoint, etc.).
Skills and Competencies:
• Capable of working with diverse client groups with varying levels of comprehension and language skills.
• Effectively manages challenging conversations and situations with respect and professionalism.
• Functions well both independently and as part of a team.
• Strong communication skills to build and maintain relationships with a wide range of contacts, including professionals and organizations in the community.
• Excellent listening and facilitation skills necessary for working with patients and their families/caregivers.
• Well-developed analytical, critical thinking, and problem-solving abilities.
• Ability to analyze, interpret, and evaluate complex information from various sources.
• Completion of a Vulnerable Sector Check within the last six months.
• Fluency in both English and French is required.
Affiliation and Compensation:
• Affiliated with OPSEU.
• Compensation is in accordance with the collective agreement between Home and Community Care Support Services Champlain and OPSEU, with salaries ranging from $54.377/hr to $63.161/hr.
• The work schedule consists of eight or nine-hour shifts totaling 35 hours bi-weekly, including one weekend per month.
• Home and Community Care Support Services Champlain offers flexible work options, including a hybrid model that combines community and clinical settings, patient homes, and remote work.
