We are seeking a highly skilled Registered Dietitian to join our team at Home and Community Care Support Services. As a key member of our healthcare team, you will be responsible for providing expert nutrition and dietetics services to patients in their homes and communities.
Key Responsibilities
- Assess patients' nutritional needs and develop personalized treatment plans to optimize their health and well-being.
- Provide education and counseling to patients, their families, and caregivers on healthy eating habits, nutrition, and disease management.
- Collaborate with healthcare professionals to ensure comprehensive care and support for patients.
- Conduct home visits to assess patients' nutritional needs and provide guidance on meal planning and preparation.
- Develop and implement nutrition programs to promote healthy eating habits and prevent malnutrition.
- Monitor patients' progress and adjust treatment plans as needed to ensure optimal outcomes.
Requirements
- Registered Dietitian in good standing with the College of Dietitians of Ontario (CDO).
- Relevant experience providing nutrition and dietetics services in a community or healthcare setting.
- Comprehensive knowledge of nutrition principles, practices, and standards of care.
- Strong communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team to achieve patient-centered care.
