Key Responsibilities:
- Conduct thorough assessments of clients, create tailored care plans, and implement and review care strategies based on the latest evidence and the unique needs of each client.
- As the primary care provider for a designated client group, you will lead their care while collaborating with other team members and support services.
- Assist clients during transitions in care by linking them with local community support services.

Qualifications:
- Current Certificate of Competence from the College of Nurses of Ontario.
- Availability to work various shifts, including days, evenings, nights, and alternate weekends.
- Up-to-date immunization records, a clear criminal background check, and a Vulnerable Sector Check are required upon hiring.
- Willingness to obtain necessary SE Health certifications.
- A valid CPR certification is mandatory.
- This role is physically demanding and requires the ability to sit, stand, walk, bend, kneel, and perform various hand movements.
