About the Role:

This is a challenging and rewarding opportunity for a Nurse Practitioner with a passion for gerontological nursing and a commitment to delivering high-quality care. As a Neuro-Behavioural Nurse Practitioner, you will work under the Senior Medical Director to support long-term care residents, specializing in managing behavioural and psychological symptoms of dementia, providing individualized care, and overseeing pharmacological and non-pharmacological treatments.

Key Responsibilities:
  • Assess, diagnose, and manage the care of residents with complex needs, including those with delirium, depression, cognitive impairment, confusion, and behavioural disturbance.
  • Develop and implement individualized care plans, working collaboratively with interdisciplinary teams to ensure comprehensive and person-centered care.
  • Provide education and support to staff, families, and residents, promoting a culture of respect, empathy, and inclusivity.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal resident outcomes.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in gerontological nursing.
Requirements:
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult).
  • CPR certification designed for health-care professionals and standard first-aid.
  • Advanced skills in the assessment, diagnosis, management, and treatment of older adults with complex needs.
  • Strong leadership, interpersonal, and teamwork abilities to motivate, evaluate, and develop staff.
  • Effective communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
